School Report Form Have your parent sign the top and then bring it to your guidance counselor.
Teacher Reference Form Give this to an English, math, science or social science teacher from 8th, 9th or 10th grade who knows you well.
Parent's Confidential Statement To be completed by your parent or legal guardian. Return with proof of income.
