The application has five parts. Please print and complete all five parts.
Student Form To be completed by the student.
School Report Form Have your parent sign the top and then bring it to your guidance counselor.
Teacher Reference Form Give this to an English, math, science, or social science teacher from 8th, 9th, or 10th grade who knows you well.
Parent's Confidential Statement To be completed by your parent or legal guardian. Return with proof of income.
Spanish Version of Parent's Confidential Statement (Documento Confidencial para los Padres)
Permission and Waiver Form To be signed by a parent or guardian.
